I understand that I must abide by these changes in order to receive food and any other items at the distribution on Thursday. I also confirm that the information I have entered on that form is accurate.

Additionally, by completing the signup form at www.swtsignup.com, Sidewalk Warriors Troy has my permission to store all information I have submitted.

Sidewalk Warriors Troy may store this information for up to twelve months. After this, any identifying information will be removed, however, the remaining demographics may be retained. This information will only be used within our organization and will never be sold, transferred, or used for any other purposes.
